First Visit Information
From the first day your child joins our office, we will work to take care of your child's dental needs. As part of your child's first visit with us, you will need to fill out a comprehensive medical and dental history for your child. These forms are important because they help us get acquainted with your child and determine the proper course of treatment.
We ask that you arrive at least ten minutes prior to your child’s appointment. This will allow time for paperwork completion and review of office policies without interfering with your participation in your child’s first dental visit in our office. If you would like to complete the paperwork at your convenience, we have them available to download and print. For your child’s first visit, we ask that you review and complete the following forms: New Patient Info and the Financial Policy.

Appointments
To schedule an appointment for your child, please call during our office hours: Monday-Thursday 8:00 am-5:00 pm and Fridays 8:00am - 4:00pm.
During the school year, the office will be open on select Saturdays 7:45am – 2pm.
We make every effort to confirm all appointments two days in advance and do require 24-hours notice to reschedule or cancel an appointment.
Transferring Records and X-rays
When requesting copies of your dental records/x-rays, we ask that the request be made in writing. Please download the
Dental Record and X-Ray Transfer Form. We will make every attempt to have the records available within 3 business days of the request. Depending upon chart size and format of the record duplications, a fee of up to $25.00 may be applied.

Payment Options
Payment for services rendered is expected at the time services are provided by the person(s) accompanying the patient unless other arrangements have been made in advance. Please see our financial policy (click the downloadable PDF above) for more detailed information regarding our financial policies.

We accept cash, check, Visa, MasterCard or Discover. As a service to our patients we will submit your insurance claims to your insurance provider. Cancellation and No Show Policy
With each appointment, time has been set aside specifically for your child. If you are unable to keep this appointment, we ask that you notify our office a minimum of 24 hours prior to the appointment. This will make the time available for another child.
If you have not cancelled or rescheduled your child's appointment, you may be charged a broken appointment fee of up to $50.00.
Snow and Inclement Weather Policy
When the weather forecast calls for snow or inclement weather that may make traveling to our office unsafe for families and staff, we will contact you about the office being closed for a portion of or the entire day. Please call us if you are unable to make it to the office or are unsure that the office will be open.
